The Behavioral Health Aide provides direct care and supportive interventions to residents, guided by Individual Services Plans and Behavior Support Plans. This role includes assisting with skill-building, crisis response, monitoring safety, and ensuring compliance with regulatory requirements. As the Behavioral Health Aide, you will not only be working as part as a cohesive and interdisciplinary resident care team, but you will also be supporting nursing services in their ongoing endeavor to deliver comprehensive, holistic health models. They will actively collaborate with team members, external partners, and residents to promote recovery and stability in a respectful and culturally sensitive manner.

Duties and Responsibilities:

The individual in this position is expected to engage in the following work-related activities.

  • Provide interventions as outlined in Behavior Support Plans and Service Plans.
  • Complete accurate and timely documentation of service provision in each resident's Electronic Health Record.
  • Read and follow Temporary Service Plans for each resident daily.
  • Deliver individual or group skills training and support in areas such as self-care, hygiene, and community resource utilization.
  • Assist residents with grooming and hygiene tasks.
  • Encourage and support residents to bathe.
  • Prompt and support residents to get their medications as prescribed. 
  • Assist with passing out meal trays and setting up trays for residents with physical impairments.
  • Monitor and support residents for behavioral health changes, escalating concerns to the care team as needed.
  • Prompt and assist residents to attend appointments and activities. 
  • Monitor and support residents experiencing behavioral health symptoms. 
  • Provide emotional support to residents experience mental health symptoms.
  • Provide feedback on the efficacy of Behavior Support Plans and Service Plans.
  • Assist residents with organizing their living spaces and perform light housekeeping tasks, including cleaning biohazards.
  • Participate in care conferences and provide input into resident care plans. 
  • Crisis Intervention and Safety
    • Assist in crisis situations by following emergency protocols and supporting traumainformed responses. 
    • Implement de-escalation techniques and non-violent interventions to promote safety for residents and staff.
    • Participate in fire safety drills and maintain familiarity with evacuation procedures.
    • Monitor and support residents expressing thoughts of self-harm, escalating concerns to the care team as needed. 
    • Document incidents promptly according to facility protocols in Incident Reports.
  • Collaboration and Communication 
    • Coordinate with oncoming shifts to ensure smooth transitions and consistent care.
    • Model and provide training to other staff regarding behavioral health interventions.
    • Attend facility-wide huddles between shifts.
    • Communicate and collaborate with other members of the Behavioral Health department and attend departmental meetings monthly. 
    • Facilitate treatment and continuity of care by collaborating with community partners, legal representatives, guardians, and other stakeholders.
    • Provide consultation and maintain effective communication with County representatives and other external agencies.
    • Uphold a dignified and caring atmosphere for residents, families, visitors, and staff.
    • Maintain a safe and secure working environment while protecting residents and community information.
    • Actively engage in training, in-services, and meetings.
    • Monitor iAlert and other communication equipment at all times while on shift.
    • Assist with projects and perform related duties as assigned.

  • Proficiency in documenting care activities, using electronic health records, and following written and oral instructions. 
  • Flexibility and creativity in time management, problem-solving, and adapting to new skills and techniques. 
  • Commitment to person-centered care, trauma-informed practices, and professional workplace conduct, including adherence to HIPAA compliance and privacy standards. 
  • Ability to work effectively both independently and as part of a team. 
  • Ability to stay calm under pressure and manage challenging behaviors with empathy. 
  • Provide compassionate, culturally competent care while maintaining professional boundaries.

Qualifications/ Requirements: 

  • High school diploma, GED, or equivalent required.
  • Minimum of two years of experience working with adults with severe ad persistent and mental illness, behavioral health needs, houselessness, or substance use disorders preferred. Job Posting- Behavioral Health Aide 
  • Ability to read, write, and communicate effectively in English. 
  • Proficiency in using computers for documentation and communication. 
  • First Aid Certification preferred. 
  • Additional certifications in mental health care, trauma-informed care, or deescalation techniques preferred. 
  • Completion of all mandated training by Oregon DHS. 
  • Adherence to work schedules, attendance requirements, and completion of required certifications and continued education. 
  • Required to wear an assigned name tag and carry community keys, an iAlert device, a walkie-talkie, and a gait belt during shifts for safety and compliance.

  • Occasionally support residents with mobility and care needs, including walking stairs (up to four flights).

  • Must frequently life and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds.

Range - $22.00 - $26.00 per hour

(shift differential from 6pm to 6am)

Schedule: Sunday through Thursday

2:00pm to 10:30pm (Swing)

or

10:00pm-06:30am (NOC)
